We have a number of programs and services available to our patients to help keep you active and healthy.

Cognitive behavioural therapy
Do you have anxiety? Consider speaking to your primary care provider about our social worker led 10-week Cognitive Behavioural Therapy Group. You will learn strategies to overcome your symptoms of anxiety.
Eligibility: adult patients of the Bridgepoint Family Health Team with anxiety.

Craving change
The Craving Change group, facilitated by trained educators, offers you an opportunity to challenge and change your relationship with food and problematic eating. Through the use of self awareness tools and cognitive behavioural strategies the team will introduce strategies to help change the way we think about eating.
Eligibility: adults, and a friend or family member who would like to join them, that identify with either problematic cravings and/or emotional eating.

Diabetes education program
Our program helps support you in managing Type 2 diabetes and pre-diabetes through self-management education, collaboration and community partnerships.

Eligibility: adults with pre-diabetes or Type 2 diabetes and their family members.
Dates and Times: Appointments available Tuesday through Thursday during regular business hours, and Friday mornings.

Health coaching
Our Health Coaching program will connect you one-on-one with a nurse in person, over the phone or by email and help you take control of your own health by choosing your health priorities, setting goals that matter to you, developing a plan that considers your strengths and barrier, and making lasting positive changes to your health.
Eligibility: all patients of the Bridgepoint Family Health Team.

Kick Butt! Smoking cessation program
Do you want to cut back on cigarettes or quit? This program provides you with one-on-one counseling and support by a nurse, and is tailored to meet your individual needs and goals. Specific topics can range from nicotine replacement therapies, medications, goal setting, and coping or behaviour strategies.
Eligibility: all patients of the Bridgepoint Family Health Team.

'Inspire' COPD Management and Support Program
Our program is designed to help support people diagnosed with Chronic Obstructive Pulmonary Disease (COPD) while working in partnership with you and your doctor. Key components of COPD management include: health promotion, proactive or preventive approaches to your care, self-management and education.
How does it work?
The COPD Program is led by a nurse practitioner and pharmacist, our team provides you with individual smoking cessation counselling, respiratory education (including assessment of COPD control, development of action plans & medication review & treatment). We offer these services to patients from the Bridgepoint Family Health Team. Initially you meet individually with the nurse practitioner and pharmacist who help develop your personalized care plan.
The nurse practitioner can help you with:
▪ COPD prevention and treatment
▪ SMART goal setting to encourage physical activity, smoking cessation, and healthy coping strategies
▪ Managing your respiratory symptoms
▪ Preventing and/or managing COPD related complications (i.e. vaccination update).
The pharmacist can help you with:
▪ Understanding how environmental triggers and physical activity impacts breathing symptoms
▪ Taking your lung medications or puffer devices safely and effectively
▪ Creating COPD treatment plans focused on your lifestyle
Eligibility: all patients of the Bridgepoint Family Health Team with a current diagnosis of COPD
