Diabetes Education Program
Welcome to the Diabetes Education Program at the Bridgepoint Family Health Team! We are committed to providing comprehensive education and support regarding diabetes prevention or management. No matter where you are in your journey, we are here to empower you to achieve your goals and improve your overall health and well-being.
Our team:
Our team consists of a Registered Nurse and a Registered Dietitian. We work closely with an Endocrinologist to support clients who have diabetes-related complexities. We provide evidence-based services that are culturally-sensitive and tailored to your needs, values, and preferences.
Who we support:
Ontario residents who are:
Age 18 and older, and
At risk for diabetes, have prediabetes, or are living with diabetes
What we offer:
Free services
Language interpretation whenever needed
Full diabetes assessment, continued follow-up, and ongoing decision support
Tailored support
Our program is designed to address the diverse needs of our clients and we provide comprehensive one-to-one support in the following areas:
Physical Activity: Exercise is an essential part of managing diabetes, and we provide guidance and support to help you incorporate physical activity into your daily routine safely and effectively.
Healthy Eating: Making healthy food choices is crucial for managing diabetes, and our program aims to support you to work towards your goals in a way that is respectful of your cultural preferences and dietary needs.
Medication Management: Managing diabetes often requires medication. We can help you understand your medications and how to take them. We will work with you and your healthcare team to ensure that your medication doses are safe and effective.
Stress Management: We understand that living with diabetes can be stressful and overwhelming. Stress can impact your blood sugar levels, and we provide stress management techniques and counselling to help you manage the stressors in your life and maintain optimal blood sugar control. We can also connect you with additional community supports and resources.
Sleep Improvement: Quality sleep is essential for overall health, and we provide education, strategies, and referrals to help you improve your sleep habits and get the restful sleep you need.
Smoking Cessation: Smoking can increase the risk of complications associated with diabetes. Our specially trained RN can offer resources, tools, and one-to-one, non-judgmental support to help you quit smoking and reduce your risk.
Foot Screening: Diabetes can increase the risk of foot problems and we offer foot screening services to help identify any potential issues early on. We also provide education on foot care to help prevent injuries, infections, and complications.
Glucose Monitoring: We provide education on glucose monitoring techniques and tools, as well as guidance on how to interpret and manage your blood sugar levels.
Future plans:
We are constantly developing our program. Plans on the horizon include fun and interactive workshops on healthy eating, mental health, physical activity, glucose monitoring, foot care, and more! Stay tunedβ¦
