If you are a current patient:

If you have information or copies of reports that you believe were not already forwarded to your physician, please send us a message and attach the reports to your message.

Alternatively, if you have physical copies of the reports, you can bring them to your appointment with you physician. The person who initially ordered the tests is the responsible clinician to discuss with you the results and implications of the test. Please book an appointment with your family physician if you need to discuss or review these reports.

If you are a third party:

Please fax us any patient reports. You can find physician fax numbers here.

Alternatively you can mail patient records on a USB or CD. You can find our clinic address on our location page.
