Sending a form directly:
If you have a copy of a form that you would like to be filled/completed by your physician, please send us a message and attach a PDF copy of the form.
Please be sure you have completed any section(s) of the form which:
need your personal information (e.g. name, address, contact, etc)
that authorize your physician to release medical information (e.g. any line that may require your own signature).
Your physician cannot complete the form until this is done.
Third-Party Forms
If there is a third-party looking to send us a form on your behalf - please request the company fax it directly to your physician, or send us a message with a PDF copy attached.
If you have already discussed cost, timeframe for completion, etc. with your physician please disregard the following comments. If you have not, the following information will likely apply:
Fees:
Most form completions are not covered by OHIP, as such there may be a fee for completion as discussed here: Uninsured Services and Block Fees. You (or the requesting party) may be sent an invoice for any applicable fees.
Time frame for completion:
Please allow plenty of time for your physician to complete your forms.
It may take approximately 2-3 weeks to complete and return your form, or send it off to the appropriate party. However, this time frame may vary depending on the complexity of the form and the type of information required to complete it. If they require more information, they may need to obtain a specialistโs opinion, or may request that you book an appointment.
