If you receive care from another healthcare provider, that you were not referred to by your Bridgepoint physician, please provide them with our contact information to ensure that they forward any reports / results directly to your physician.
If you were seen by another health care provider and have questions, or now need to see your physician in follow up, please book an appointment.

Kindly note that there is no need to advise your physician, as we should receive the consult report from the specialist.
I was seen at an Emergency room
When you register at a hospital, please let them know who your family physician is so that the hospital can send reports to your physician. The hospital does not have this information on file without you letting them know. We do not always receive reports from the various emergency rooms in the province.

Please remember that you have signed an enrollment form with your Bridgepoint Physician. Any care delivered by a walk-in-clinic within 100km of Toronto will result in outside use charges to the team. If you would like to advise us of your visit, and provide any information that you know was not already forwarded to your physician by the third party, you can send us a message

If you have information or copies of reports that you believe were not already forwarded to your physician, please send us a message and attached the report to your message.
Alternatively, if you have physical copies of the reports, you can bring them to your appointment with you physician. The person who initially ordered the tests is the responsible clinician to discuss with you the results and implications of the test. Please book an appointment with your family physician if you need to discuss or review these reports.
