The Bridgepoint Family Health Team provides comprehensive primary care to more than 6000 patients from the Toronto Riverdale community and across the GTA. Our team uses an innovative collaborative care model that includes you, the patient, as a full partner in your care.

We are located on the  Bridgepoint Campus and are affiliated with Sinai Health.

We bring family physicians together with interdisciplinary health care professionals - including a nurse practitioner, registered nurses, social workers, pharmacist and diabetes educators.  We work as a team to provide you with the best patient care possible.
